When a club or activity starts, we will send out messages to students and post them on the morning announcements.<\/p>\n
To register your student to play sports, families will need to login to their Parent Portal account, and click on the \u201cschool store\u201d<\/strong>.<\/p>\n Click on the\u00a0specific sport(s) and school<\/strong>\u00a0your student will register\/play for.\u00a0 For more detailed information please see below.<\/p>\n List of things to make sure you have completed before your student(s) are able to participate:<\/strong><\/p>\n Parent Infinite Campus<\/strong>: Username: Email Address \u00a0\u00a0\u00a0\u00a0Password: (can click on \u201cForgot Password\u201d)<\/p>\n Once you have signed your portion<\/em> – Under \u201cSchool Store<\/strong>\u201d please click on \u201cMY CART<\/strong>\u201d and click \u201cSUBMIT<\/strong>\u201d once this is done, have your child log in to their Student Infinite Campus for their portion.\u00a0 (You will receive a confirmation e-mail once your paperwork has gone through.)<\/p>\n Student Infinite Campus<\/strong> \u2013 Username: Student ID Number\u00a0\u00a0\u00a0\u00a0\u00a0 \u00a0Password: Password student uses to log in to their Chromebook<\/p>\n Click \u201cDocuments\u201d \u2013 List of sports documents will be listed, have them click on each one and scroll until they can click sign document, submit them and form shows \u201ccomplete\u201d.<\/p>\n Physical form and Alternate Physical form can be found above by clicking on \u201cClick here for athletic forms\u201d.<\/p>\n If your student athlete\u00a0HAS NOT<\/u><\/strong> received an athletic physical in the last calendar\u00a0year (valid physicals are those after 4\/1\/2020) they must try and get one completed ASAP and return the first Physical form to us (2).\u00a0 爱奇艺资源站 Urgicare can provide athletic physicals for $40.<\/p>\n If your student athlete HAS<\/u><\/strong> had an athletic physical and you have previously submitted a doctor signed form to the main office in the last calendar year (valid physicals are those done after 4\/1\/2020) then you just need to fill out the Alternate Physical Form either electronically or via paper form.<\/p>\n Physicals or Alternate Physicals can be faxed to 262-359-2184, emailed to rcarter@kusd.edu<\/a> or dropped off to the main office Monday-Friday 8-3:30pm. <\/strong><\/p>\n If you have any questions please feel free to call the office at 262-359-2240.
